Percutaneous mitral valve repair: an overview of new interventional therapies and associated outcomes
نویسنده
چکیده
Percutaneous alternatives to conventional surgery for mitral regurgitation have emerged over the last decade with devices modeled after existing surgical techniques. The most developed of these catheter approaches include leaflet repair and annuloplasty. A device that plicates the free edges of the mitral valve to create a double orifice repair is the only leaflet-repair method in development. This technology has completed a randomized trial, but results have yet to be reported. Annuloplasty devices include indirect approaches via the coronary sinus and direct approaches via retrograde left ventricular implantation. There is less human experience with annuloplasty devices than leaflet repair. Leaflet repair has been used in both degenerative and functional mitral regurgitation, while annuloplasty devices are used for annular dilatation associated with functional mitral regurgitation. There is obvious appeal for less invasive approaches in patients who are at high risk of surgery, and ongoing trials will define their ultimate utility in less high-risk patients.

Right sided heart evaluation after successful mitral valve replacement.
Introduction: It is well-documented that right-sided heart dysfunction and significant tricuspid valve regurgitation (TVR) have adverse effects on patient outcomes after left-sided heart valve surgery. Therefore, the evaluation of right ventriclular (RV) function and TR severity in patients who had undergone mitral valve replacement (MVR), associated with/without concomitant su...

Nonsurgical treatment of clinically important mitral regurgitation (MR) has evolved tremendously over the past decade. Recent studies of percutaneous mitral valve repair procedures have shown that less invasive procedures are safe and can be effective in selected patients. MitraClip has been studied most extensively. The MitraClip is attached to the middle scallop of the mitral leaflets by a tr...

Heart failure has become a real epidemic condition related to poor outcomes despite advances in medical therapies. Prevalence of significant mitral and/or tricuspid regurgitation is high in patients with advanced heart failure. Novel transcatheter techniques have recently emerged as a minimally invasive alternative in patients deemed high-risk for surgery or inoperable. Among them, MitraClip® s...

Percutaneous mitral valve therapy became a reality more than 25 years ago with the first description of balloon valvuloplasty for rheumatic mitral stenosis.1 More recently, percutaneous closure of paravalvular leaks after surgical valve implantation has been shown to have a potential, but limited, role.2–4 However, valvular mitral regurgitation (MR) remains largely the purview of surgery. Recen...

Until recently, percutaneous catheter therapy for valvular heart disease was limited to catheter balloon valvuloplasty for aortic, mitral, or pulmonic stenosis. A number of new approaches to percutaneous valve therapy are now developing rapidly, including methods for catheter-based valve replacement and repair. Stent-mounted valve prostheses have been successfully implanted in the pulmonic and ...
